Agreements

Basic Agreement

By responding “Yes” to an email with the the bullet points listed below you'll be digitally signing, acknowledging, and agreeing to the following:

• I am 18 years of age or older.

• I am not currently married.

• I am not currently using or addicted to alcohol, drugs, or any other substance. If I do begin to use any of these things, I will pause or cancel my services with Nate J. Clark. I understand that in order to continue with Nate J. Clark I’ll need to be sober from them for at least one month so I can continue to make uninhibited progress.

• I am not currently addicted to pornography or any other sexual practice.  If I do begin to view or participate in any of these things, I will pause or cancel my services with Nate J. Clark. I understand that in order to continue with Nate J. Clark I’ll need to abstain from them for at least one month so I can continue to make uninhibited progress.

• I take full responsibility for my words and my actions and will hold Nate J. Clark and his practice harmless for anything that happens as a result of coaching.

• I understand that this is educational coaching, not counseling, and in no way offers a diagnosis or treatment of depression, anxiety, or any mental health issue.

First Session

First Time Agreement

• Everything in the Basic Agreement

• I agree to pay Nate J. Clark $60 for my first session and for the services outlined under “First Session” in the pricing section of his coaching page.

• I understand that If I cancel an appointment I will only receive a 70% refund. Rescheduling will not result in any fee.

Additional Sessions

"Up to 3 More" Agreement

• Everything in the Basic Agreement

• I agree to pay Nate J. Clark $120 for each additional session (limited to 3) for the services outlined under “Additional Sessions” in the pricing section of his coaching page.

• I understand that If I cancel an appointment I will only receive a 70% refund. Rescheduling will not result in any fee.

Monthly Subscription

Subscription Agreement

• Everything in the Basic Agreement

• I agree to pay Nate J. Clark $400 by the 5th of each month for the services outlined under “Monthly Subscription” in the pricing section of his coaching page. If I begin accepting his services after the fifth day of my first month, I agree to pay a prorated amount for that month and $400 for any months following.

• I understand that I can cancel anytime.

• I understand that I will not receive a refund for any portion of the $400 once services for the current month have begun.
